Incident, Accident
& Hazard Reporting
When something goes wrong
Side A
If This Happens – Do This
  • 1Make it safe first. Stop safely, hazard lights, protect passengers.
  • 2Anyone hurt? Call 000. Give first aid only within your training.
  • 3Notify Operations as soon as immediate risks are handled.
  • 4Leave things as they are if safe. Photos if safe. Witness names and numbers.
  • 5Complete the Incident Report in Mitti before the end of your shift.
  • 6Near miss? Same Incident Report, same shift. Hazard? Hazard Report in Mitti – as soon as you can.
Serious injury or danger
Call 000 immediately. People first, paperwork second.
Deadline: end of shift
Every incident is reported in Mitti no later than the end of the shift it happened in. Student behaviour: by the end of that journey.

2 Purpose

This SOP tells you what to do when an incident, accident, near miss or hazard happens – and how to report it.

3 Applies To

All drivers on TBS services – TBS employees and contracted drivers alike (not the drivers of self-managed operators, who follow their own procedures) – for anything that happens on the road, at a stop, or in a depot: collisions, injuries, near misses, hazards, aggression, property damage, spills. Steps marked Supervisor are performed by the on-duty supervisor or the Operations Team.

4 Scenarios – Trigger → Action

Find the situation that matches, and do the steps in order.

Scenario A – Collision or Injury
  • 1
Driver Stop safely. Hazard lights on. Protect your passengers from further danger.
  • 2
Driver Check for injuries. Serious injury or danger: call 000 immediately.
  • 3
Driver Give first aid only within your training. The onboard first aid kit is behind your seat area.
  • 4
Driver Notify Operations as soon as immediate risks are addressed.
  • 5
Driver Leave everything where it is if you safely can – do not move the bus or debris until police or Operations say so. Take photos if it is safe.
  • 6
Driver Collect details: witness names and contacts; for another vehicle: the driver’s name, phone, licence number and address, their insurer, and the rego, make, model and colour – the Incident Report asks for every one of them.
  • 7
Driver Complete the Incident Report in Mitti – parked, park brake on, seatbelt off; on your phone or on the bus tablet – before the end of your shift at the latest. Serious injury or danger: 000 first, Operations immediately, report as soon as you safely can.
  • 8
Supervisor Enter the incident into the Operations Management (OM) system. Notifiable incidents are reported to the regulator by TBS within legal timeframes.
Do not admit fault at the scene
Swap details and stick to what happened. Never say “it was my fault” or “sorry, I didn’t see you” – TBS and the insurers work out fault later, not you at the roadside.
Scenario B – Near Miss
  • 9
Driver Report it the same shift – using the same Incident Report in Mitti, before the end of the shift in which it happened.
Why near misses matter
A near miss is the same event as an accident, minus luck. Reporting it lets TBS fix the cause before someone is hurt.
Scenario C – Hazard or Unsafe Condition
  • 10
Driver Can you remove the danger without risk to yourself – a bag in the aisle, a loose panel, a small spill you can cover? Do it. If not, keep people away from it until help arrives.
  • 11
Driver Report it in Mitti as a Hazard Report as soon as you can – today, not tomorrow.
  • 12
Supervisor Pass the hazard report to the relevant manager and the HSE Team for assessment. Put immediate controls in place where needed.
Which report? Ask yourself these questions, in order
  • Is it the bus, and it cannot go on? → Call Operations first. Then Breakdown in Mitti – continue in SOP-07 · Breakdown
  • Is it the bus, but still safe to drive? A minor fault, damage, a warning lamp → Defect in Mitti, before the end of your shift – how: SOP-11 · Report
  • Not the bus, and it has already happened? A crash, an injury, aggression or a threat, a near miss → Incident Report in Mitti, before the end of your shift.
  • Not the bus, not happened yet – but it could hurt someone? A spill, a damaged stop or road, an unsafe place to load, faulty depot equipment → Hazard Report in Mitti, as soon as you can.
Always – The Reporting Rules
  • 13
Driver Deadline: report every incident before the end of the shift it happened in. Student behaviour (SOP-05) is recorded sooner – by the end of that journey. Name the student and the school if you know them, and say what you did to manage it.
  • 14
Driver Be factual. What you saw, heard and did. No guesses, no blame.
  • 15
Driver You may be asked to help the investigation. Cooperate – investigations exist to prevent the next incident, not to punish honest mistakes.
No reprisal
TBS maintains an open reporting culture. You will never be disadvantaged for reporting incidents, near misses or hazards in good faith.
